Request a Check - Fidelity (2024)

You can request a single check or schedule checks to be sent automatically on specific dates. Checks can be sent to your record address or to your bank account.

Your check will arrive five to seven business days after your request is received by Fidelity. There is no fee to have a check sent, however if you need to liquidate any holdings, commissions or mutual fund fees, if any, still apply.

  • Personal Withdrawal Service (PDF) . This page will open in a popup window. (for brokerage and mutual fund accounts)
  • College Investing Plan Distribution Form (PDF) . This page will open in a popup window. (for qualified or non-qualified distributions from your 529 plan, as well as rollover distributions to another 529 plan provider)
  • Health Savings Account (HSA) Redemption (PDF) . This page will open in a popup window. (for distributions from your HSA account)

Important Tax Considerations

Before you request withdrawals from your retirement accounts, note IRS regulations regarding early or normal distributions and minimum required distributions (MRD). Guidelines are included with the form. Consult a tax advisor regarding your circ*mstances.

For Single Withdrawals

  • Request a check online
  • IRA Single Withdrawal Request Form (PDF) . This page will open in a popup window. (for early or normal distributions as well as a minimum required distribution(MRD))
  • IRA Return of Excess Contributions (PDF) . This page will open in a popup window. or Request a Check Online (for removing excess contributions from your IRA)
  • Complete your IRA Excess Contribution Online
  • Fidelity Retirement Plan Single Withdrawal Request Form (PDF) . This page will open in a popup window. (for early or normal distributions as well as a minimum required distribution (MRD) from your Fidelity Retirement plan, including Keogh Profit Sharing, Money Purchase and Self-Employed 401(k) plan options) This form is NOT for Fidelity 401(k)s, 403(b)s, or other employer-sponsored retirement plans held through your employer. For a withdrawal from your Employer-Sponsored Retirement Plan (such as a 401k or 403b) Single Withdrawal Request (You will be directed to NetBenefits. Once you log into NetBenefits, choose the account from which you want to withdraw. Then click on Loans, Rollovers, and Withdrawals.)

For Regular (scheduled) Withdrawals
