What is reimbursed

  • I am going to a contracted healthcare provider
    The healthcare will be reimbursed in the same way as in the Netherlands. What is not reimbursed

  • Extra benefits such as a single or double room, or a phone, TV, internet access, or a refrigerator in your room.
  • Additional costs of physicians who charge rates other than the statutory rates in the country in question, such as non-contracted physicians, private healthcare providers, or a Professor (Germany). They often charge high, private rates. In most cases, these additional costs will not be reimbursed.
  • Additional (bank) charges for a payment by you or CZ to or from an account outside the Netherlands.
  • Additional costs due to exchange rate fluctuations. We always reimburse healthcare costs in euros. If the invoice was paid in a foreign currency, your reimbursem*nt may be higher or lower than you expected. If your healthcare provider outside the Netherlands sent their invoice directly to us, you may in some cases receive an invoice from us for the exchange difference.

Request permission for:

  • Healthcare that you would also have to request permission for if you were to get it in the Netherlands.
  • Treatment involving medicines that are on the requesting permission for medicines abroad list(in Dutch).
  • Healthcare that is on the requesting healthcare abroad list(in Dutch). This includes treatments involving admission for one or multiple nights.

If you go to a contracted Belgian or German healthcare provider, you will not need prior permission for any admissions. If the treatment is on this list(in Dutch), please call us on . We will then tell you whether this treatment is covered by the contract we have with the hospital of your choice and whether or not you need to request permission for it.

You must always wait for our permission before starting your treatment. Reimbursem*nt of the treatment or medicine is subject to our permission.

Use the application formto request permission. Please send the completed form to us by post. In most cases, you will receive a response within 10 working days. We may occasionally ask you for extra information. If your request is granted, we will upload your statement to 'Mijn CZ'.
